Community Support Incorporated can accept Requests for Service by mail or facsimile. We welcome Case Managers to discuss their referrals over the telephone with the CSI Operations Manager or the Senior Service Co-ordinator before forwarding a hard copy.
Our team of experienced Service Coordinators are available to take your referrals and discuss service provision, on (08) 8112 8000.
Community Support Incorporated aims to at all times meet the individual requirements of our service users, and is committed to confirming these requirements prior to service provision commencing.
To send a referral :
Please Note:
- A full Request form is required for a new consumer or a change in support services.
- A Rollover Request assumes that nothing has changed from the previous Request for Service and that the service is to continue without changes, directly from the previous request.
- When dowloading the electronic version in Microsoft Word™, the form has been designed so that the fields on the form can be directly typed into (interactive fields). If you would prefer to have these emailed to you or you are having problems with the forms, please contact us on 8112 8000.
- If downloading the pdf version, please complete and fax or post to us.
