Publications and Insurance

When you apply to become a CSI Support Worker, you will receive a Support Worker Information pack, which includes documentation such as:

  • CSI Application Form (CSI Approved/Employees only)
  • National Criminal Record Check Consent Form (requiring an accompanying 100 points of identification)
  • Statutory Declaration (to be completed if born outside of Australia)
  • Support Worker Insurance information
  • Support Worker Acknowledgement form
  • Support Worker Motor Vehicle Inspection Checklist (to be completed by mechanic)
  • Direct debit bank details form
  • ABN form
  • A prescribed CSI invoice pad
We require from applicants photocopies of your Driver's Licence and motor vehicle registration receipt, along with any certificates of qualifications, e.g., Senior First Aid, Manual Handling, Cert III or higher in Disability or Aged Care Work.
